This farmer decided he wanted a better yield. Robert came to our clinic in November of 2021 with a plan. Fix his thinning hairline and stop wearing hats everywhere he went. Having done his research, he knew a large hair transplant would be in order. We decided that a 3,000 graft F.U.E. procedure would be enough to fill in the hairline with enough left over to density the lacking midzone. Luckily for Robert, his donor site (the DHT-resistant area located at the occipital scalp) is very strong. This means that he could have a large 3,000 graft procedure without a noticeable esthetic effect on the donor area. This procedure was not a small one. This gentleman had the option of moving forward with a smaller 2,000 graft procedure which would have helped to reform his hairline but would not be as impactful and dense as the results you are seeing here. This can be a tough decision for our patients. Yes, a larger procedure means a longer and more expensive one, but in terms of recovery it's almost identical. How many grafts do I need for my restoration? This is the golden question. Often patients will submit a photo or example of a hairline and ask us to estimate the total number of grafts. This sounds like an easy task for an experienced hair transplant surgeon but there are so many factors at play that without a proper meeting, discussion, and surgical plan the range is vast. Why does the hairline use up such a large number of grafts? When we are creating the hairline in an area with low to no density we not only have to create a dense border, but the supporting hair behind the frontal hairline must appear thick to give the look of a full head of hair. Furthermore, the front requires singles. This is a term for a graft of hair that contains one follicle. While some grafts contain up to four hairs the frontal hairline naturally grows with "singles" along the first three rows of hair. Behind this we may use a mix of single and multi grafts to add extra density.

This young man came to us hoping to stop his frontal hairline recession and restore areas with little to no hair remaining. In the before photo you'll notice some markings that were created during this gentlemen's pre-operative appointment. These sections show where the grafts will ultimately be placed, but also act as a guide to determine 𝐰𝐡𝐚𝐭 𝐭𝐲𝐩𝐞 𝐨𝐟 𝐠𝐫𝐚𝐟𝐭 we are using. The area at the front (closest to the eyebrows) is where single follicular units will be placed. This means a graft that will only ever grow one single hair at a time. Something you may not know is that some hair follicles contain multiple roots, as many as four. These grafts tend to naturally grow behind the frontal hairline and around the edges of the hair. It's simply how mother nature designed our hair and to give someone a natural looking result we must follow her lead. The second area back is where doubles and triples will be placed with the final sections containing only triple and quadruple grafts.

Getting a youthful hairline back doesn't take as long as you might think. This gentleman came to our clinic hoping to get back his youthful appearance by addressing his fairly severe hair loss. Focusing on the frontal hairline and crown we placed over 3,000 grafts to achieve this impactful result. Over half of the grafts were focused along the frontal hairline with a smaller portion placed at the crown (better visible in the third photo). For this procedure we did not place any grafts into the midzone, found between the frontal hairline and very top of the head. One reason for this was to save a majority of the grafts for the most visible section located at the front.

You may notice that the density behind the hairline is a bit lacking. This was a calculated decision and due to Dr. Sharma's harvesting technique we still have plenty of viable grafts should this man desire a second transplant to restore an even larger area of hair. The after photos were taken 7 months after his hair transplant and are showing roughly 65% of the final result in terms of density and growth. Over the next year he will see the hair begin to thicken, and even new hairs sprouting to help give greater density to the recipient area. In all, this is a massive success and goes to show that using a high number of grafts is essential to obtaining dramatic results. For those hoping to achieve similar hair restoration progress please note - if only 1,500 or 2,00 grafts are used for a case like his, the result would reflect that decision.

Here is a fantastic case of strategic placement having a huge impact on overall density. This gentleman had a very large area of hair loss at the crown which was beginning to creep forward into the midzone. He was also noticing some recession along the fronto-temporal angles which are near the temples. He opted to address both with one large procedure rather than go through multiple recovery periods. While I don't always recommend this, his expectations for the finished result were very grounded.

In January of 2019 I met Cole and the one thing I recall was a very handsome face obscured by a rather unsightly baseball cap.
